This is the 2018-2019 Concord Youth Hockey Travel Player Tryout and Evaluation Registration Form.
This registration is required for players who wish to try out / be evaluated to participate on a CYHA travel team for the 2018-2019 season.
To register you will need to have a profile for your child on our website (you will only need to do this once)
